Your first visit is longer than a standard appointment so nothing gets rushed.
Book online
Pick a day and time that works. Same-week appointments are usually available.
Complete intake
Fill out the forms from home in about five minutes. Bring your insurance card and a list of medications.
Meet your doctor
A calm, unhurried visit. You leave with a clear plan and concrete next steps.
Before your first visit
Bring what you need for a smooth check-in. No paperwork to print, but it helps to have these on hand:
- Insurance card and a photo ID
- A list of current medications and doses
- Recent lab results or records from other doctors, if you have them
- A list of questions you want answered
Transferring your records
Once you sign a records release, we handle the request to your previous provider for you. To speed things up, you can also email recent records to hello@harborviewfamilymed.com.
Insurance & billing
Coverage is verified before your first visit. Co-pays are collected at check-in and billing questions are handled in the office.

Common questions
How soon can I get an appointment?
Most new patients are seen within the week. Same-day sick visits are available when you call before 10 AM.
Do you see children?
Yes. Dr. Vance and Maya Chen care for patients of all ages, from newborns through adults.
What if I do not have insurance?
Call the office to ask about self-pay rates and payment plans. No one is turned away for an inability to pay.
Do I need a referral to book?
No. Harborview is a primary care clinic, so you can book directly.
Can I move my ongoing care to Harborview?
Yes. We will help transfer your records from your previous provider and continue any current prescriptions.
